Beth Israel Lahey Health Primary Care (BILHPC) is building a high performance primary care model and establishing a diabetes education program to provide quality care and medication-assisted therapy for patients with diabetes in an outpatient setting. The Diabetes Educator Team Nurse partners with providers, pharmacists, nutritionists, and other care team members to educate, support, and improve health outcomes of patients with diabetes. Responsibilities include coordinating and facilitating clinical management of outpatient populations, collaborating with community providers, providing telephonic triage and assessment, managing INR for patients on Coumadin, administering vaccines and medications, performing nursing activities such as wound care and point of care testing, monitoring test results, managing clinical supplies, providing oversight and education to unlicensed staff, ensuring patient safety and compliance with privacy regulations. Specific diabetes education duties include clinical and psychosocial assessment, identifying educational needs, developing and implementing care plans, empowering patients with knowledge and skills for diabetes management, educating on medication administration and monitoring, supporting preventive health behaviors, conducting quality improvement studies, and providing educational sessions and tools for care teams. Required qualifications include graduation from an accredited Nursing Program (Bachelor's preferred), 1-3 years related experience, strong interpersonal and patient education skills, proficiency with Microsoft applications, RN or LPN license, Basic Life Support certification, Certified Diabetic Care Educator and Specialist (CDCES), and CPR certification. Preferred qualifications include experience with diabetes technologies. The role requires medium physical work and adherence to vaccination requirements. BILHPC is a network of over 80 practices in Eastern Massachusetts and Southern New Hampshire, providing comprehensive primary care services.
